Outstanding work in IT-enabled change in healthcare winners

This project alerts a keyworker when a known cancer patient has an emergency admission to hospital. Patients are flagged on the trust's IT system, Lorenzo, and this generates an email to the keyworker, which can be accessed from a computer or smartphone. The keyworker can then make sure that the patient is in the care of an appropriate team for speedy but supported discharge.

The Ipswich Hospital NHS Trust has delivered, in partnership with Kainos, a clinical information portal known as Evolve. The project's primary aim was to free information, so the right treatment can be given efficiently and effectively, according to the very latest patient case notes. It has already delivered a financial return by removing costs from the business, and is inspiring other trusts to follow a similar path.

Urgent Care Clinical Dashboard - NHS Bolton and the Department of Health Informatics Directorate

The Urgent Care Clinical Dashboard provides Bolton GP practices with near real-time information on unscheduled care activity. The information is displayed through a graphical, user-friendly dashboard, enabling practices to proactively manage and improve the quality and safety of care provided to patients.
