Winner: Telemedicine to deliver 24/7 stroke thrombolysis, East of England Strategic Health Authority

The project uses unique computer technology, developed by the NHS regional IT experts, to link A&E departments at any hospital with a specialist stroke consultant anywhere. The consultant uses a very high quality video and audio link to see and speak to the patient and to view CT scans before making the prompt diagnosis needed if thrombolysis is to be given.

DrThom is the UK's first online doctor service to register with the Care Quality Commission. In partnership with Lloyds pharmacy, DrThom offers patients a care-pathway from home to pharmacy.

Managing Leicestershire COPD patients at home, Acute Respiratory Team, University Hospitals of Leicester

A 12 month telehealth project, carried out by the respiratory department at Glenfield Hospital, Leicester using Docobo's doc@HOME, aimed to increase self-management and avoid hospitalisations in 40 patients with COPD. Data was monitored and interventions were made by clinical teams. Hospital admissions were saved and patients and their carers reported improved experience, quality of life and confidence in their ability to self manage their disease.

Birmingham OwnHealth and Assistive Technology, NHS Birmingham East and North

The Assertive Case Management Service cares for patients with long term conditions, encompassing a self care model which optimises quality of life and educates patients regarding their condition. Parameters are set by specialist staff and key questions detect any deterioration in the patient's condition, raising an alert when necessary for a face-to-face response by a clinician.
