
Patients with chronic illness often have multiple functional and psychosocial problems that are hard to identify in busy clinic consultations, in both primary and secondary care sectors. Through an IT partnership a Patient Concerns Inventory has been developed which provides the model for the next generation of IT-assisted, holistic, patient-empowered consultations.
Staff of the obstetrics and neonatal departments at Mid Cheshire Hospitals NHS Foundation Trust wanted to enhance the care and experience of parents of preterm or poorly babies. Sometimes mothers are unable to see their baby for 24-48 hours, so webcam technology has been deployed to enable the parents to 'see' their baby from the mother's bedside.

The West Cheshire Health and Care portal is a web-based one-stop shop where local people can find information without needing to know in detail how the health and care 'system' works.
